Prof Terence Stephenson - Introduction

Describes his role as Paediatric Advisor to the National Patient Safety Agency (NPSA) since the death of Wayne Jowett from an accidental intrathecal injection of vincristine in 2000 and the subsequent “Organisation with a memory” report following this. He also describes his experience of the strong safety culture in Shell Oil that he observed and how it is enforced and contrasts this to his observations of patient safety in the UK.
